This membership plan is a dental savings plan and is NOT a dental insurance. This plan cannot be used with dental insurance or any other discount plans.
No annual maximums, deductibles, claim forms, waiting periods, missing tooth clauses, or pre-existing condition exclusions.
The plan ends 1 year from the date of enrollment.
Annual membership must be paid in full at the time of enrollment.
Enrollment fees are subject to change annually without notice.
No refunds will be issued at any time if patient does not utilize the dental plan.
Unused benefits do not roll over to the following year.
